Meningococcal disease is caused by the bacterium Neisseria meningitidis. Many people carry it harmlessly in the nose and throat, but occasionally it invades the bloodstream or the lining of the brain, causing septicaemia or meningitis. It is uncommon, and it is feared because of its speed: a child can go from seeming unwell to critically ill within a day. Several groups of the bacterium exist, which is why schedules use more than one vaccine, typically MenACWY and MenB.

Symptoms

  • Fever, often with cold hands and feet
  • Severe headache and a stiff neck in older children
  • Dislike of bright light
  • A rash of small red or purple spots that does not fade when pressed
  • In babies, a bulging soft spot, a high-pitched cry, refusing feeds or being floppy

How it spreads

Spreads through close, prolonged contact with respiratory droplets or saliva, such as living in the same household, coughing or kissing.

Why it's serious

This is a rare illness, and most fevers are nothing like it. What makes it different is the pace: the advice everywhere is that if a child is deteriorating quickly, or has a rash that does not fade under pressure, they need urgent medical assessment rather than watchful waiting.

  • Septicaemia, blood poisoning that can damage limbs and organs
  • Hearing loss, a common lasting effect of meningitis
  • Brain injury affecting learning and development
  • Limb amputation after severe septicaemia
  • Death, which remains possible even with prompt treatment

Most at risk: Babies and toddlers have the highest risk, with a second peak in teenagers and young adults. Risk is also higher without a working spleen or with certain immune conditions.

Treated in hospital with intravenous antibiotics, started as soon as it is suspected rather than after tests confirm it. Close contacts are usually given preventive antibiotics.
